Family Legacy Benefit Application

Benefit

Subject to the conditions and limitations stated herein, WoodmenLife will pay a member benefit to assist with the care of an Orphan of members in the sum of $1000.00 per month (“Benefit”).

Conditions and Limitations

  1. Except as set forth in Section 2, all individuals, including the Orphan, residing in the Orphan’s household on the date of death of the last surviving Parent must be a Good-Standing Benefit Member. An Orphan who is not a Good-Standing Benefit Member and less than six (6) months old on the date of death of the last surviving Parent may be eligible to receive this benefit provided that the Orphan becomes a Good Standing Benefit Member within six (6) months of the date of death of the last surviving Parent.
  2. All individuals residing in the Orphan’s household on the date of death of the last surviving Parent who are not Good-Standing Benefit Members must have (i) applied for a WoodmenLife benefit certificate and (ii) subsequently been either denied or table rated 4 or higher.
  3. Except as otherwise set forth in this section 3, the Benefit payments will cease when the Orphan reaches nineteen (19) years of age. Subject to all other conditions and limitations, an Orphan receiving the Benefit that is and remains enrolled full-time (i.e. no less than twelve (12) credit hours per semester) in a post-secondary education institution accredited by an accrediting agency recognized by the U.S. Department of Education and maintains the institution’s equivalent of a “C” average is eligible for the Benefit until the Orphan reaches twenty-two (22) years of age.
  4. Each Parent of the Orphan must have been a Good-Standing Benefit Member for at least one (1) year prior to his or her date of death.
  5. The date of death of the last surviving Parent must occur prior to the Orphan reaching eighteen (18) years of age and the application for the Benefit must be received by WoodmenLife within one (1) year of the date of death of the last surviving Parent.
  6. The Benefit will commence the first day of the month following approval of an application for this Benefit.
  7. An Orphan receiving the Benefit who marries or enlists with the United States Army, Marine Corps, Navy, Air Force, Space Force or Coast Guard, will no longer be eligible for the Benefit and payments will cease immediately upon the occurrence of either such event.
  8. WoodmenLife reserves the right, in its sole discretion, to make the Benefit payment directly to the Orphan or to a relative, guardian or other individual responsible for the Orphan’s care and support. The Benefit is intended to be used solely for the health, education, maintenance and support of the Orphan.
  9. In circumstances where the Orphan’s biological father is not identified on the Orphan’s birth certificate, the biological father shall not be considered in determining the Orphan’s eligibility for the Benefit.
  10. WoodmenLife reserves the right to review each application on a case-by-case basis and, in its sole discretion and without waiving its right to enforce each and every condition and limitation set forth herein in the future, approve or deny an application.


Definitions

  1. Good-standing Benefit Member – a WoodmenLife member who has made all payments according to the terms of the benefit certificate.
  2. Orphan – an individual both of whose Parents are deceased.
  3. Parent – the biological or adopted mother or father of an individual.
